Accepted Insurance

If your insurance requires you to get a referral from your primary care physician, please provide us with the referral before your visit. We can confirm if we participate in your medical insurance plan and your options if you have a co-payment. We aim to make the payment process easy and convenient for all our patients from Springfield, Westfield, Longmeadow, and Northampton, Massachusetts, and nearby communities.

Types of Insurance We Accept

We accept a variety of insurance plans for procedures that are deemed medically necessary. If your plan requires a co-payment, you will need to make that payment at the time of your visit. We will assist you by submitting a claim to your insurance carrier after your visit. Please remember that any charges not covered by your insurance plan are your responsibility.

Referrals

If your insurance requires you to get a referral from your primary physician, please provide us with the referral before or during your visit. If your insurance carrier has not authorized your visit, you can reschedule your appointment or sign a waiver accepting the financial responsibility for the entire charge.
